Many of today's routers implement SIP ALG (Application-level gateway), This feature is usually enabled by default. While SIP ALG could help in solving NAT related problems, the fact is that many routers' SIP ALG implementations are wrong and break SIP.
Most if not all VoIP providers I have come across recommend you disable SIP ALG. And, So do I.
