That is a very vague question and as such you cannot expect a concise or definitive answer.

The most significant impact on SIP is not private or public networks. Rather it is whether the end to end path includes Network Address Translation. NAT is often encountered at the boundary between a private network and the public internet.

SIP was designed with little or no attention paid to NAT. Put 'SIP NAT STUN' into a search engine and read away to your heart's content.
